It sure looks to be printed with the use of a computer to get the type in the right places and very neatly also.

A bit of information found on the Hawaii DOH website:

Vital Statistics

Phone: (808) 974-6008

The Hawaii District Health Office provides information on accessing/amending vital records (birth, death, marriage, and divorce certificates), information on getting married in Hawaii, processing Voluntary Establishment of Paternity (VEP), and registration of home births.

That date stamp on the rear reminded me of a time clock stamp.
