It can be friendly fire or self inflicted as long as there is enemy fire?

Is that correct?

Some guy is saying that his uncle slipped in the shower and got a PH for it.

What is the exact criteria for qualifying for a PH?

You've got it right. It needs a Casualty Report and a Comban Action Report signed by his CO! Neither exists for 02 Dec 69.

Plus, of course, treatment records!

The possibility, although remote, would involve others writing or approving of injury reports and after action reports. If he were in a base camp and being mortared at the time, I could see it happen. I can't say it did, but I can't say it didn't.
