Maybe O'Reilly's ego is that big to think that one missed interview with HIM sank Kerry. Gore didn't interview with O'Reilly in 2000, and he only missed the Presidency by 537 votes.
To be fair, Kerry was avoiding LOTS of interviews with the press during the campaign. He seemed to get in a lot of soundbites from his speeches, but never exposed himself to tough questions.
I don't think that O'Reilly's "almost-interview" with Kerry surrogates did Kerry any good. O'Reilly did ask them tougher questions than he asked the President, and they seemed unprepared to answer. Kerry IS a good debater, good at dancing around an issue, but the surrogates didn't seem clear about Kerry's position. Maybe they hadn't gotten the latest daily update!
But O'Reilly probably did under-estimate the importance of the SBVT!

Yes he did, amongst other reasons. Though he did not say "The Factor" itself, but you damned well knew he meant that. Talk about Ego.
